Adds require_protocol_slashes so callers can reject https:example.com and require https://example.com. It defaults to false to keep existing behavior.

npm test passes: 328 tests, all builds, and lint. Checked the option in the generated Node, ES, browser, and minified builds too.

Closes #2937.
